Purpose: Use iPrescribe to add and manage diagnoses in a patient's record or while prescribing. You can search by ICD-9 or ICD-10 code, save favorites for quick access, and update diagnosis status at any time.
Audience: Active iPrescribe users
You can add and manage diagnoses to patient records in iPrescribe. Once you add a diagnosis, you can view it in the patient record and reference it when creating prescriptions. You can also switch the coding system from ICD-9 to ICD-10 in the patient's Diagnoses section.
Add a Diagnosis
1. Open the patient profile.
2. Scroll down to the Diagnoses section and tap Add.
3. On the Diagnoses screen, tap a Diagnosis Codes tab to select the appropriate ICD coding system. Your settings determine which coding systems appear.
4. Type a diagnosis in the search field and select the appropriate diagnosis code. If you don't find the diagnosis you're looking for, select a different diagnosis code tab and the list updates to show the new search results. Select the diagnosis from the results.
- Green checkmark: Indicates that the diagnosis is already assigned to a patient in the system.
- Yellow star: Indicates that the diagnosis has been selected as a favorite and shows in the Favorites tab.
5. On the 'Diagnosis Details' screen, enter an Onset Date if applicable, then tap Create Diagnosis.
6. A confirmation message appears, and the new diagnosis displays in the patient's Diagnoses section.
You can also add a patient's diagnosis while writing a prescription within the Rx Details screen.
1. Select the + sign in the Primary Diagnosis field and select a diagnosis with the correct diagnosis code on the Select Primary Diagnosis screen.
2. View the diagnosis and diagnosis code on the Rx Review screen, then select a secondary diagnosis or continue prescribing.
The new diagnosis displays in the patient's Diagnoses section.
Manage an Existing Diagnosis
You can resolve, deactivate, reactivate, or delete a diagnosis from the Diagnosis Details screen.
1. In the patient profile, tap on an existing diagnosis in the Diagnoses section.
2. The Diagnosis Details screen displays management options.
Use this when a condition has been treated or is no longer active.
1. On the Diagnosis Details screen, tap Resolve Diagnosis.
2. The diagnosis moves to Inactive Diagnoses and shows a "resolved" status.
Use this to temporarily remove a diagnosis from the active list without deleting it.
1. On the Diagnosis Details screen, tap Deactivate Diagnosis.
2. A confirmation message appears: "Diagnosis has been deactivated."
3. The diagnosis moves to the Inactive Diagnoses section.
Use this to restore a resolved or deactivated diagnosis to the active list.
1. In the patient profile, tap Show Inactive / Resolved Diagnoses.
2. Tap on the inactive diagnosis to open the Diagnosis Details screen.
3. Tap Activate Diagnosis.
4. A confirmation message appears: "Diagnosis has been activated."
5. The diagnosis returns to the active Diagnoses section.
Use this to permanently remove a diagnosis from the patient's record.
1. On the Diagnosis Details screen, tap Delete Diagnosis.
2. A confirmation dialog appears. Tap Delete to confirm or Cancel to go back.
Note: Deleting a diagnosis is permanent and cannot be undone.
You can save frequently used diagnoses as favorites for quick access.
1. After searching for a diagnosis, tap the star icon next to the diagnosis you want to favorite.
2. The star fills in to indicate the diagnosis has been added to your favorites.
Favorited diagnoses appear in the Favorites tab for rapid selection.
3. To un-favorite a diagnosis, select the yellow star, then confirm in the pop-up that you want to remove it from the favorites list.
